2. Co-op Declaration,
3. Health Aid Statement and Undertaking,
4. Co-op Clarification Text,
5. Co-op Explicit Consent Statement,
- Please fill in the documents and submit your CV and ID photocopy to the Office of the Internship and Co-op Directorate (Mahmutbey Campus, Block A, Floor Zero, Room No. 5).
- Application Form must be signed in original by the advisor.
- The Co-op Assessment File should be submitted to your advisor after completion.
